Social Worker

Avoca, IA: (Jan-21-08) The Iowa Board of Social Work brought charges against Avoca social worker David Benedix, alleging that he falsified client records, accepted fees by misrepresentation, violated rules related to social work practice, and engaged in unethical conduct detrimental to the public. Records show that Benedix co-owned and operated Phoenix Family Services, and in May, 2004, the Iowa Department of Human Services served Benedix with a notice that an audit of Phoenix records identified numerous instances in which Phoenix was overpaid by the department due to lack of documentation and falsification of client records.

As part of a settlement reached, Benedix agreed to a lifetime suspension from providing rehabilitative treatment and supportive services in an owner, managerial or supervisory capacity. Additionally, he also was ordered to pay back $19,615 to the United States for overpayments due to fraud. Recent records show that Benedix voluntarily surrendered his license, agreeing to not practice social work and not to represent himself as a licensed social worker in Iowa.
