At the age of 14, Victoria Petersilka hanged herself after being allowed to enter a bathroom alone on February 9, 2002. The original report of her death contained statements that Victoria "was going to kill herself" and also had a check mark in the box stating "suicidal/homicidal ideation." A later version of the report did not include such a statement, and the box was empty. Victoria's family filed a lawsuit against the institution that alleged Maryville workers failed to properly supervise Victoria and accurately report signs of suicidal behavior. They awarded the family a $650,000 wrongful death settlement. (Jun-29-05) [SUN TIMES]
