Veteran Medical MalpracticeConcerns have been raised about the conditions in Veteran Administration (VA) hospitals. Poor cleaning practices and even vermin that could potentially spread infection have been found in some VA hospitals. Inspectors have found that some staff members are not properly supervised. False reports have been filed by some employees claiming essential medical devices like defibrillators have been checked and maintained, when in fact they have not. Supervisors have been found missing when they were supposed to be working, and supervisors have been accused of giving their computer passwords to resident physicians, allowing them to cosign their own reports without supervision.

Veteran Affairs Hospitals fail Veterans At least 63 cases of malpractice have been found between 1997 and 2002, that resulted because of the failure of a supervisor to oversee residents. Miami, FL: In a scathing editorial, the Miami Herald reveals that the VA hospital in Miami where VA colonoscopies took place was warned by the Department of Veteran's Affairs about potential problems with colonoscopy equipment. According to the newspaper the Hospital checked its procedures and reported everything fine—that is, until a more extensive review later found problems. [ 
Miami, FL: It's a medical malpractice case in the making, after it was revealed that as many as 10 veterans have tested positive for infectious liver disease allegedly caused by contaminated colonoscopy equipment. What's more, an email sent to the Associated Press (AP) by the Veterans Affairs Department (the VA) on March 19th revealed that tubing attached to the endoscopic equipment was cleaned at the end of each day, rather than immediately after each patient—a violation of the manufacturer's instructions. Washington, DC: An investigation relating to glitches in the electronic medical records of the nation's veterans has uncovered the potential for incorrect VA drug doses and other veteran drug doses at many veteran hospitals. While it has not been proven if any patient was ever harmed as a result of the error, the revelation is troubling in light of a nation-wide push towards the wholesale adoption of electronic medical records.
