Heather Doll on Botox is now covered by insurance for migraines. I think the most important part of this treatment is finding a doctor who is specifically trained in this treatment for this condition. The botox website (manufacturer) has links to doctors trained in use for migraines. I took the list and found the doctor with the most experience (in years) and then googled him with the word "reviews" to find out what other patients said about him (may not always be true - could be a nutty person writing in or his friends writing good things so use your good judgement).
